Teacher Survey on Technology Experiences
We are professors in the School of Education at Indiana University. We are inviting you to participate in a research study to determine both the methods by which you learned to use technology in your teaching, and your thoughts about the best ways to use technology in education.
We are requesting your participation, which will involve completing the questions on the following pages. There are 21 questions in total. Completing the questions should take no more than 20 minutes.
There are no foreseeable risks to you associated with completion of this study. This study may, however, provide information that will assist teacher educators and state and local policy makers in improving the types of technology experiences provided to pre-service teachers.
The data collected from this study is confidential. The questionnaire is anonymous. The results of the study may be published but your name will not be known.

Your participation in this study is voluntary, you may refuse to participate without penalty. If you decide to participate, you may withdraw from the study at any time without penalty and without loss of benefits to which you are otherwise entitled. If you withdraw from the study before data collection is completed your data will be returned to you or destroyed.
